III. It isn't necessary to have a website to add your name to the fanlist, but if you do please save a code to your own server and link it back to this fanlisting ... and please, no direct linking!
IV. Fill out the form as completely as possible. Your name, email, & country are required fields per Fanlisting Network rules - without them I can't add you!
